The Care Manager will play a key role in engaging individuals at the highest risk of experiencing or contributing to violence and supporting their journey toward safety, wellness, and stability. This role involves relationship-building, service coordination, and long-term support to help participants reach their goals and reduce cycles of harm and trauma in our communities. 

This position may require evening and weekend hours and involves fieldwork in community-based settings. FORCE Detroit is committed to fostering a supportive work environment and offers opportunities for professional development.

Key Responsibilities

  • Coordinate and provide safe, timely, client-centered care that is equitable, effective, and trauma-informed.
  • Develop, implement, and regularly monitor individualized service plans for program participants.
  • Build strong, trust-based relationships with participants to support their growth and connection to needed resources.
  • Facilitate care coordination with family members, schools, health providers, and social service agencies to ensure comprehensive support.
  • Serve as a consistent advocate for participants, supporting them in navigating systems and making informed choices about their care and goals.
  • Maintain regular communication with community partners to improve referrals and collaborative service delivery.
  • Arrange and track referrals to social services, healthcare providers, legal services, workforce development programs, and other supports.
  • Conduct regular follow-ups to monitor participant progress and adjust plans as needed.
  • Maintain accurate and timely documentation of all participant interactions, service plans, and case notes in compliance with program standards.
  • Produce regular reports on participant outcomes and progress.
  • Promote quality and cost-effective interventions to achieve meaningful participant outcomes.
  • Available to work flexible hours, including evenings or weekends, in the event of a program-related crisis or emergency situation.
  • Collaborate closely with internal teams and external partners to deliver wraparound services.

Qualifications

  • Minimum of 2 years of experience working with individuals and families experiencing trauma, crisis, or violence-related challenges.
  • Strong interpersonal and communication skills; able to build rapport and trust across diverse communities.
  • Knowledge of crisis intervention and trauma-informed care practices.
  • Demonstrated understanding of Detroit’s neighborhoods, systems, and community dynamics.
  • Experience working in a nonprofit, social work, or public health setting is preferred.
  • Ability to work independently and as part of a multidisciplinary team.
  • Must possess a valid Michigan driver’s license, reliable transportation, proof of insurance, and a good driving record.
  • No current criminal cases or convictions for domestic violence (within 10 years), sexual assault, or child abuse.
