  • Support the interoperability platform and patient-identification model development.

  • Ensure data use agreements are in place and adhered to.

  • Determine and address technology and data security needs as they arise.

Travel

  • Regular travel from SHF headquarters in Marquette to partner sites.

  • Represent SHF at meetings, conferences, and other program-related travel as needed.

  • Highly organized and detail-oriented.

  • Must present the highest level of ethics and professionalism in verbal, written, and in-person interactions and possess exceptional interpersonal and communication skills.

  • Able to handle sensitive and confidential program and financial information with discretion.

  • Must possess a forward-thinking, solutions-oriented, and customer service-oriented mindset and be able to proactively solve problems and think analytically.

  • Familiarity with rural health, chronic disease management, or Community Health Worker models is a plus.

  • Experience with grant compliance and reporting, or willingness to learn MDHHS/CMS reporting requirements.

  • Proficiency with project management and productivity software (e.g., Monday.com or similar platforms).

  • Availability to travel regularly, by personal vehicle, across the pilot area; valid driver’s license required.

  • Punctual, dependable, and self-directed, with a positive attitude.

Education & Experience

  • Required: Bachelor’s degree in public health, health administration, social work, nonprofit management, or a related field, or equivalent professional experience.

  • Experience in program or project management, ideally within a nonprofit, public health, or grant-funded setting.

  • A passion and enthusiasm for nonprofits, philanthropy, and health are a plus.
